2. Schedule an Interview: We believe in getting to know each potential member personally.

We will schedule an interview with you to better understand your interests, goals, and how C.A.S.E. can support your unique needs.

 

3. Gather Required Documentation: To complete the admission process, you will need to provide certain documentation, including:

• Proof of identity (e.g., driver's license or passport)

• Medical records and emergency contact information

• Any relevant educational or vocational records

• Current contact information for your support network

• If receiving DDA Services, please provide CCS, Support Broker Information, Person centered plan and budget. 

 

4. Attend an Orientation: After your interview, we will invite you to attend an orientation session. During this session, you will learn more about our programs, available resources, and the opportunities for community engagement that await you.

 

5. Customize Your Plan: At C.A.S.E., we value individuality and choice. Together, we will develop a personalized plan that aligns with your interests, preferences, and goals. You will have the flexibility to choose the activities and experiences that matter most to you.

 

6. Begin Your Journey: Once your admission is complete and your personalized plan is in place, you can officially start your journey as a member of the C.A.S.E. community. You will have access to our programs, resources, and a supportive network of fellow members.

 

7. Stay Engaged: Active participation and engagement are key to making the most of your experience at C.A.S.E. We encourage you to explore our diverse range of activities and events, forge new connections, and continue your personal growth.
